Custom Tables Questions

For my prevalence study I have a number of frequency tables and have the
following questions:
1. Any idea why the note on comparison testing that was at the bottom of
tables has disappeared?
i.e. Note: Values in the same row and subtable not sharing the same
subscript ....
2. Is there a way to use syntax to insert footnotes?
3. Is there a way to insert a blank line(s) in a table?
