I have not yet used CODEBOOK. But try this.
go to the "variables view" and see if you have a label for zero.
If not enter one via the variables view.

When I run Frequencies (Analyze --> Descriptive
Statistics --> Frequencies) I can see all the codes, but
when I run Codebook (Analyze --> Reports --> Codebook)
the code 0 is not reflected in the output.
Can anybody please suggest an explanation? Is this a bug?
