one- and a four-year survival with cox regression

How would I calculate a one- and a four-year survival with a cox regression?

My cox regression model contains (apart of status ant time) various covariates (some of them categorical) and a pattern variable with 3 categories.

I would like to calculate the percentage of one- and a four-year survival per pattern category.
