Hi Paul,

While I have not formally used SA in my line of work, I will take my shot
at your question.  I am assuming that all of your customers did enter the
study on the same day, therefore, some of your right-censored customers
may have stuck around for a month, 5 months, 2 years, etc.  Therefore, all
you need are two columns (time in months, and an indicator of churn).  For
those who have not churned, the time is simply the length of time they
have been around since your study has 'ended', with the indicator set to
zero for no event (assuming 1 indicates churn).
